Attachment E: Community of Practice Assessment First Email Reminder

Dear NSSP Community of Practice Member:

You were recently invited to participate in the NSSP Community of Practice member satisfaction and needs assessment. If you have not already completed the assessment, we encourage you to take a few minutes to do so. We value your opinion and appreciate you setting aside the time to respond, as your feedback will help improve the member experience and inform resource needs for future community of practice activities.

The deadline for completing the assessment is [date].

Completing the assessment is voluntary and takes approximately 15 minutes. Please complete the assessment in a single session as you will not be able to save it and return to it later. CDC and CSTE will not publish or share any identifying information about individual respondents or health departments, and results of the assessment will be aggregated and shared with CDC and the NSSP Community of Practice membership via a final report, webinar, and/or publication.
