Dear NSSP Community of Practice Member:

This is a final reminder that the member satisfaction and needs assessment for the NSSP Community of Practice will close on [DATE]. We thank those of you who have already responded to the assessment.

If you have not completed the assessment, please take a few minutes to do so. Your feedback is critical, as we strive to meet the needs of the community of practice membership.

The deadline for completing the assessment is [date].

Completing the assessment is voluntary and takes approximately 15 minutes. Please complete the assessment in a single session as you will not be able to save it and return to it later. CDC and CSTE will not publish or share any identifying information about individual respondents or health departments, and results of the assessment will be aggregated and shared with CDC and the NSSP Community of Practice membership via a final report, webinar, and/or publication.
