Thank you for participating in the National Syndromic Surveillance Program (NSSP) Community of Practice
(CoP) assessment. Your feedback is important and will be used to make improvements and plan future
activities of the CoP.
The assessment is voluntary and takes approximately 15 minutes to complete. Please complete the
assessment in a single session as you will not be able to save it and return to it at a later date.
Results of the assessment will be aggregated and shared with the NSSP CoP membership via a final
report, webinars and/or publication. CDC and ISDS will not publish or share any identifying information
about individual respondents or health departments.
Thank you for your participation.

CDC estimates the average public reporting burden for this collection of information as 15-minutes per response, including the time for
reviewing instructions, searching existing data/information sources, gathering and maintaining the data/information needed, and
completing and reviewing the collection of information. An agency may not conduct or sponsor, and a person is not required to
respond to a collection of information unless it displays a currently valid OMB control number. Send comments regarding this burden
