This is a reminder that you have one week until [date], to provide input regarding current public health ethics practices at your health department.
We have invited state and local public health directors like yourself to participate in an online survey that will take approximately 11 minutes. Your feedback is valuable and will be used to develop better resources for health departments to improve their ability to respond to ethical issues that arise in public health practice. In order to preserve your privacy, we will not link your name or the name of your health department to your individual responses in our report of the findings. You may designate another representative from your health department to complete this survey if that person is more involved in ethics related activities.
Even if you are not implementing public health ethics activities at your health department, we are interested in your feedback.
