Dear [Name]:


You recently completed an online survey conducted by the Public Health Ethics Unit of the Centers for Disease Control and Prevention regarding your health department’s public health ethics activities. On that survey you indicated that you would be willing to participate in a telephone interview to provide more information about how your health department is addressing ethics issues that arise in public health practice.


We are contacting you to schedule this interview, which we anticipate will take 60 minutes to complete. We are hoping to conduct the interview by {DATE}.


While we have some specific questions we would like to ask, this is not intended to be a structured interview. We hope that our questions lead to an open discussion about:

  • Your department’s current public health ethics activities, such as training or committee meetings

  • Your department’s needs regarding public health ethics resources

  • Your department’s perceptions of the impact of implementing public health ethics activities



Your answers will help improve the public health ethics resources, training, and guidance that the Public Health Ethics Unit at CDC provides to health departments. In order to preserve your privacy, we will not link your name or the name of your health department to your individual responses in our report of the findings.
