Subject: Pre-Notification: CDC’s Survey, State of State, Territorial and Tribal Suicide Prevention: An Environmental Scan

As you are aware, suicide remains an important and unfortunately, growing public health problem. From 2000-2016, suicide rates increased nearly 30% in the United States. To reverse these trends, a national goal was created to reduce suicide rates by 20% by 2025. To this end, The Centers for Disease Control and Prevention’s (CDC) Division of Violence Prevention, along with partners and our contractor, Global Evaluation & Applied Research Solutions (GEARS), Inc., is conducting a web-based survey titled, State of State, Territorial and Tribal Suicide Prevention: An Environmental Scan. This survey will be administered to state, territorial, and tribal (S/T/T) suicide prevention coordinators (or their equivalents), grant project directors, and suicide prevention coalitions supporting S/T/T suicide prevention efforts, in all 50 states, Washington, D.C., 5 U.S. Territories, and among 15 tribal grantees. As a subject matter expert in suicide prevention in your S/T/T, we invite you to join this critical work by completing this survey.
Survey responses will be used to identify (S/T/T) factors that may be related to increases and differences in suicide rates across the country, to inform improvements in suicide prevention strategies, to guide potential future funding opportunities for S/T/Ts, and to help meet the nation’s goal of reducing suicide rates 20% by 2025.
The survey includes information about the problem of suicide in your S/T/T, associated risk factors, suicide prevention infrastructure, suicide prevention plans, partners, prevention activities, and facilitators and barriers to prevention. All data will be stored securely throughout the data collection, analysis, and reporting periods and all results will be reported only in aggregate form.
You will receive the survey link and instructions via email in three days. While your contribution and expertise is crucial to the success of the project and to achieving our national goal, participation is voluntary.
