Two weeks ago, the CDC's Division of Violence Prevention in the National Center for Injury Control and Prevention and Global Evaluation & Applied Research Solutions (GEARS), Inc. invited you to complete a survey to help the field better understand factors that may be associated with suicide rate increases across states, territories and tribes (S/T/Ts). This information includes questions about the problem of suicide in your S/T/T, organization and infrastructure to address the problem, suicide prevention plans, implementation partners and champions, suicide prevention efforts, risk factors, and facilitators and barriers to prevention. CDC will use the information to inform improved strategies, materials, and potential funding opportunities to improve public health practices and reduce suicide rates to meet the nation’s goal to reduce suicide rates 20% by 2025.

All data will be in a secured environment throughout the data collection, analysis, and reporting process and all results will be reported only in aggregate form. The survey will take approximately 30 minutes to complete. Please use this link to access the survey: [insert link].

The survey closes in two weeks. For your answers to be included in the findings, the survey must be completed by [INSERT DATE].
