Leadership Development - Change Request Form
This form must be completed by credit unions requesting a change in the project or
training topic for the 2017 Leadership Development grant.
NCUA will not consider approving reimbursement for any topics or projects not listed
in the approved grant application. If a change to the approved project/training topics is
needed, complete this form and submit it to [email protected]. The email subject
line should be “Leadership Development Change Request” and include the grant
commitment number of the award.
NCUA will respond to the change request by email. All change requests require
approval in writing.
