Your household has been chosen by chance from among all U.S. households. We would like someone in your house to participate in an important study. The study is being done by the Centers for Disease Control and Prevention (CDC). This is a telephone study of up to 16,000 people. The study is about health and injuries. Men and women over the age of 18 will be asked questions about their experiences. The data we are collecting are very important. They will be used to guide national policies.


We are writing you because we have found that many people like to know ahead of time that they will be called. The letter is addressed to “Resident.” We do not know your name. We do not keep address information in our files. Being part of this study is voluntary. People may refuse to answer any question or stop at any time. The answers you give us will be kept private. Answers from interviews will always be in summary form. No one will ever know how anyone in your house answered the questions. No one will ever be able to figure out the names of people who were in this study.


A person from RTI International will call your house soon. RTI International is a non-profit research group that works for CDC. To learn more about RTI International, visit www.rti.org. The interview will take about 25 minutes. If we call at a time that isn’t good for you, please tell us. We will be happy to set up a better time. If someone in your house is eligible for the study and completes the interview, we will send them a $10 check in appreciation.
