Under the Individuals with Disabilities Education Act discretionary grants are authorized to support technology, State personnel development, personnel preparation, parent training and information, and technical assistance activities. This grant application provides the forms and information necessary for applicants to submit an application for funding, and information for use by technical reviewers to determine the quality of the application.

The time required to complete this information collections is estimated to average 26.5 hours per response, including the time to review instructions, search data resources, gather the data needed, and complete the collection. The purpose of this change worksheet is to include the OMB approved"Evidence Form" (1894-0001) instrument in the Part D Discretionary Grant Application Package-IDEA under the approved OMB control number (1820-0028). Grantees previously provided evidence data, however the use of the evidence form will allow a more concise and clear collection of the evidence data; there is a program change increase of 1200 annual burden hours which accounts for the level of data elements on the standard evidence form versus providing an open ended/non-structured narrative.
