Other Changes/Justification Statement: HUD is requesting a change request to include forms HUD-93742 and HUD-93742a that were erroneously omitted from the last approval in 2016. The forms were acknowledged in the Supporting Statement but due to an oversight were not uploaded in ROCIS. There are no changes or revisions. The Agency is requesting an expedited review, if possible.
