The DD-2990 and DD-2991 forms are used to ensure DoD personnel deployed in support of Operation United Assistance are promptly evaluated for exposure(s) to the Ebola virus within 12 hours prior to departing an Ebola outbreak country or region (West Africa). Ebola is a Quarantinable Communicable Disease as named in Executive Order 13295 and may necessitate quarantine and isolation to protect the individual and the health of the public.
