The Commission intends to discontinue information collection 3060-1249 once this information collection is approved.

Short Statement: Due to the transfer of the information collection requirements from OMB control 3060-1249 and the re-evaluation of the existing burdens for this information collection, the Commission makes the following adjustments: the annual number of respondents has increased by +1,562, from 3,510 to 5,072 annual number of respondents; the annual number of responses has increased by +3,619, from 3,680 to 7,299 annual number of responses; the annual burdens hours has increased by +5,562 annual burden hours, from 5,260 to 10,822 annual burden hours; and the annual cost has increased by +$9,200, from $1,600 to $10,800. There are no program changes to this collection.
