[Mathematica Policy Research/Decision Information Resources] and the Food and Nutrition Service of the U.S. Department of Agriculture are doing a study to test some innovative strategies made in the Supplemental Nutrition Assistance Program (SNAP) Employment and Training (E&T) program in your state to help people find and keep jobs. About a year ago, while enrolling in SNAP employment and training services, you filled out paperwork, including a consent form, to participate in this study.

As you might remember, part of the study involves some short surveys by telephone over the next few years. This letter is to inform you that an interviewer from [Mathematica Policy Research/Decision Information Resources] will call in the next few weeks to ask you to do the first survey, for which you will receive a $20 gift card after the survey is completed. However, if you call us before [DATE] and complete the survey, you will receive a $40 gift card.

The survey will take about 30 minutes. All answers you give will be private, except as required by law. Your participation is voluntary and will not affect any government assistance you may receive now or in the future. The purpose of the survey is to learn more about how the new SNAP E&T services provided in your state are affecting the people who use them.
