The grantee project director should complete the questions below every six months.


All grantees complete Questions 1- 16.

Tier 1 grant recipients replicating the SMARTool would complete 1 - 25.

Tier 1 grant recipients replicating the TAC would complete 1-16 & and 26 – 29


Score 0 for each item you mark “not at all”, 1 for each item you score as “in progress” and 2 for each item you score as “fully implemented” Your total score is what will be entered into the web-based performance measures reporting system.
