Hello, Mr./Ms. [PARTICIPANT’S LAST NAME], my name is [INTERVIEWER NAME] and I’m calling from Westat, a survey research firm, on behalf of the Food and Nutrition Service, United States Department of Agriculture. A few weeks ago we sent you a survey to ask you some questions about food and shopping. We have not received your completed survey and would like to do the survey now over the phone. Completing the survey is voluntary. If you decide not to participate, there will be no penalty or loss of benefits.
3. I would like to speak with the primary food shopper in your household. The primary food shopper is the person who does the most grocery shopping for your household. Are you the primary food shopper in your household?
YES ………… 1 Go to Question 5
NO ………….. 2 Go to Question 4
4. May I speak with the primary food shopper in your household?
If contact made with primary shopper, repeat 2 above and continue to question 5
