NHTSA Form 1454 - Teen Recruitment Invitation briefly describes the study, the incentives, and provides the web address for those who want to participate.
NHTSA Form 1455 – Parent/Guardian Consent Form briefly describes the study and the requirements of participation. The form is administered via the Internet before any data are collected from the participant.
NHTSA Form 1456 - Teen Agreement to Participate describes the requirements of participants. The form is administered via the internet before any data are collected from the participant.
NHTSA Form 1457 - Teen Data Collection Prior to Training includes questions about the participant including contact information and driver’s license number (which is necessary to obtain crash and citation data at the end of the study). The remaining questions ensure that the person meets study inclusion criteria.
NHTSA Form 1458 - Six-Month Follow-Up Teen Contact is a letter reminding participants that they agreed to complete follow-up testing 6-months after the initial training, that they will receive an incentive for doing so, and the web address for the testing.
NHTSA Form 1459 - Six-Month Follow-Up Data Collection contains questions about driving exposure and any crashes or citations during the first six months of driving. The questionnaire is administered during the on-line 6-month follow-up session.
NHTSA Form 1470 – 7-Day Trip Log asks participants about each trip mileage over a one-week period in order to provide a measure of driving exposure.
NHTSA Form 1471 – Teen Reminder Card is sent to teens who have not completed the training within 10 calendar days following the initial invitation to request that they complete the training.
