Abstract: Authorization to apply for a no-fee passport and/or request for a visa is granted to those who can verify U.S. citizenship and legitimate official travel needs. The information collected on this form is shared with the Department of State (DoS) and the embassy of the country to which the traveler is going for the production of the needed travel documents. Respondents include DoD civilians and military members and their dependents traveling on official orders.
