E-Mail Recruitment and Consent for Participation in Brief SO/GI Data Collection Assessment

In 2017, CDC’s Division of STD Prevention (DSTDP) received provisional approval from the Office of Management
and Budget (OMB) to collect separate data elements for Sexual Orientation (SO) and for Gender Identity (GI) for
cases of sexually transmitted diseases reported through the National Notifiable Diseases Surveillance System
(NNDSS). Beginning in January 2018, these data elements were added to the implementation guide for all
jurisdictions with instructions for how to include SO/GI data available locally in case notifications reported to CDC.
These data elements were listed as “Optional” in the implementation guidance.
Participation in this telephone survey is strictly voluntary; no incentives or compensation are offered for
participation. There are no penalties for non-participation. Additionally, no identifying information that could be
used to identify you or your reporting jurisdiction will be included in the aggregate analysis and reporting of the
information gathered trough this brief telephone survey.
