9	Questionnaire page meant to inquire regarding focus of type of registering organization with numerous options	Now a landing page called the “Annual Survey asking only about name of respondent, state, sponsoring organization, zip code, name of sponsor POC, and nine options to choose from (including other with a blank that can be filled in for something “other”) instead of the numerous listing from previous form.

Non-emergency activities used for if any? Other community outreach? Funding sources and percentages? Annual CERT budget?
