There are many ways that the collaborative (coalitions or partnerships) develop and function dependent upon the various needs of the collaborators and the communities they operate in and serve. In order to assess the processes that are developing among members of your collaborative and the potential outcomes of your work, we ask that you complete the following questions on an annual basis.
