versions of forms for OMB 2900-0020. VA shall also comply with 38
USC 5126 in its processing of these forms. In its next revision,
extension, or reinstatement request, VA will explain why a guardian
or POA cannot sign these forms.

The VA Form 29-336 is used to
designate a beneficiary and/or optional settlement. The VA Form
29-336a is used as an additional page to the 29-336 if the Veteran
decides to list more than three beneficiaries or designate
beneficiaries on a different policy. The information on the form is
required by law, Title 38, USC Sections 1917, 1949 and
1952.
