Recently we mailed your household a letter asking you to complete a short survey about your household for the Health and Injury Survey, sponsored by the Centers for Disease Control and Prevention (CDC). We have not yet received a response from your household to the survey request.

For this study to be successful, it is important that we hear from every selected household. Results from this study will be used to inform and guide national prevention efforts.

We are asking that one adult in your household fill out the survey using the link provided below. To ensure that the study represents different types of people, please ask the «youngest/oldest» adult «male/female» living here to complete the survey in the next two weeks. If there are no adult «males/females» that live here, please ask the «youngest/oldest» adult «female/male» to complete the survey.

It will take less than 5 minutes to complete the initial survey and to learn more about the next steps for this study.
