This household has been chosen at random to participate in a study by the Centers for Disease Control and Prevention (CDC) about health and injuries in the United States.

We are writing you because we have found that many people like to know ahead of time that they will be called.

• Your household was selected to represent others like it in the country. Your participation is important because it is not possible to talk with everyone in the U.S.

• We do not know your name or keep address information.

Being part of this study is voluntary, and you may refuse to answer any question or stop the survey at any time. The answers you give will be kept private, and No information that could personally identify you will be given to CDC or anyone else. Your answers will be combined with responses from others who are in the study when the results are presented.

In the next few days, you will receive a call from Westat, a social science firm working for the CDC (www.Westat.com).

• Our caller ID will say Westat, so you know who is calling. The person calling you will be polite and professional.

• The survey will take about 30 to 40 minutes to complete. If we call at a time that isn’t good for you, please tell us, and we can set up a better time.

• When we call, we will select an adult age 18 or older living in this address to participate and will send them «$10/$25» in appreciation for their time in answering our questions.
