MA-100A RECEIPT OF PRODUCER MILK (PINK TICKETS)
The MA-100 Receipt of Producer Milk Tickets is used in conjunction
with the MA-100A Producer Weight Record Bulk Farm Tank Tickets.
The same disclaimer is used for MA-100 and MA-100A tickets. The
following changes will be made when the lab orders new MA-100 Pink
Tickets.
1) The words: “Form Approved, OMB 0581-0032" in the upper righthand corner of the front page of the form;
2) The words: “Exp. XX/XXXX” printed beneath and after (1)
3) The Burden and Non-discrimination Statements appear on the front
page of each form, on a separate page, or on the reverse side of the
first page of the form. These paragraphs must appear in a minimum
font size of 8. The following are the current Burden and Nondiscrimination Statements:
According to the Paperwork Reduction Act of 1995, an
agency may not conduct or sponsor, and a person is not
required to respond to a collection of information unless it
displays a valid OMB control number. The valid OMB
control number for this information collection is 05810032. The time required to complete this information
collection is estimated to average 15 minutes per
response, including the time for reviewing instructions,
searching existing data sources, gathering and
maintaining the data needed, and completing and
reviewing the collection of information. OMB approval
expires for this form on March 31, 2020.
In accordance with Federal civil rights law and U.S.
Department of Agriculture (USDA) civil rights regulations

and provide in the letter all of the information requested in
