PLEASE FOLLOW INSTRUCTIONS CAREFULLY BEFORE SUBMITTING
• This form will be used to collect nominations for JCOC 93 and 94.
o JCOC 93: March 29-April 4, 2020
o JCOC 94: July 12-18, 2020
• Participants are expected to attend the conference in its entirety.
• This form MUST be typed. Forms that are not typed will not be considered.
• A one-page biography must be submitted with this form. Biographies should be no more than 300 words.
Resumes, Curriculum Vitae, or letters of endorsement will NOT be accepted.
• Only completed nomination forms with biographies will be accepted.
• Nomination deadline: Friday, December 20, 2019. Extensions will not be granted.
• Those selected to attend JCOC will be informed of the locations, which will be revealed at registration.
• Department of Defense nominating authorities should submit their nominations to Service points of contact, so
that the POC can screen all nominations before sending to the JCOC program office.
• JCOC Alumni nominations should be emailed to: [email protected]. To ensure the
form reaches the program office, the subject line should include “JCOC 2020” and the nominee's first and last
name. (e.g., "JCOC 2020 John Doe.")
• The JCOC program manager will screen nominations for eligibility and then forward to a selection panel for
scoring. Selection announcements are expected by mid-January 2020.
• Nominators will be informed of selection results and are responsible for notifying their nominees of the
outcome.
