OMB Control No. 3095-0071, NA Form 13172, Selective Service System Records Request
In March 2020, two changes were made to NA Form 13172 - the addition of a Request Purpose field and an email address field. The purpose of these changes is to ensure the receiving technician understands the reason for the request and to allow an increase in e-billing and electronic delivery of copies requested.
