Sexual Risk Avoidance Education (SRAE) Grantee Needs Assessment

The Family and Youth Services Bureau (FYSB) is interested in learning about the status of SRAE grantee needs. This needs assessment is being conducted by the SRAE Training and Technical Assistance (T&TA) team with guidance from the Family and Youth Services Bureau.

The assessment is voluntary and you will need 15 minutes to answer the questions below. The information provided will be kept private and will not be used to make funding decisions, or other grantee-specific decisions. This information will provide the T&TA team insight into the most prevalent needs and requests across all SRAE grantees (Title V State, Title V Competitive, and General Departmental).

Please note that your participation in this assessment is voluntary. Assessment responses are anonymous and will be kept private. THE PAPERWORK REDUCTION ACT OF 1995 (Pub. L. 104-13) Public reporting burden for this collection of information is estimated to average 15 minutes per response, including the time for reviewing instructions, gathering and maintaining the data needed, and reviewing the collection of information. An agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number.
