The Federal Communications Commission
(Commission) requires telecommunications carriers and other
providers of telecommunications to contribute to the universal
service fund and other funds. Contribution revenue data, as well as
other information, are reported by carriers on FCC Forms 499-A
(annual) and 499-Q (quarterly). Accompanying these forms are
instructions on how to report revenue.

The Commission is reporting
adjustments to this information collection. The number of
respondents has increased from 6,700 to 6,900, the total annual
responses decreased from 41,650 to 41,250 and accordingly burden
hours are being adjusted from 247,375 hours to 252,025 hours, a
difference of an additional 4,650 hours annually as a result of the
number of entities affected and based on the Commission’s
historical experience with contributor reporting. There are no
program changes.
