I am writing to ask you to take part in the CDC Health Survey, administered under the direction of the US Centers for Disease Control and Prevention (CDC).


The CDC is conducting this survey to learn more about the public health problems that affect Texas residents. Completing the survey will help us improve public health and preventative services for you, your family and your community. {We enclosed an insert with one sample question from the Health Survey, to give you an idea of what the survey is about.}


This survey should be completed by the adult (who must be age 18 or older) in your household who has the next birthday.

However, it is important to the success of the study that everyone chosen takes part. We expect the survey to take approximately 15 minutes. We know your time is valuable and have enclosed $2 as a token of appreciation. {If the adult with the next birthday completes this survey, they will receive an additional $5.}.


We can assure you that your individual privacy will be maintained. The information you provide will be combined for all participants and reported only as summaries. You will not be individually identified, and your personal information will not be shared unless required by law.

Complete and mail-back the enclosed paper survey and receive $5.

OR

Complete and return the survey online (instead of by mail) and receive $10.
