Attachment F- Reminder Email
Dear Public Health Laboratory Mycobacteriology Supervisor or Designee,
Recently, an email was received asking you or a designee to participate in a voluntary questionnaire titled, “Pandemics, Natural Disasters and other Interruption of Service Events: Contingency Planning Considerations for Mycobacteriology Testing.”
The purpose of the assessment is to allow feedback to be provided to the Laboratory Branch concerning 1) the availability of public health laboratory (PHL) and specifically mycobacteriology laboratory continuity of operations plans (COOPs), 2) the classification and number of events that interrupted PHL and mycobacteriology testing services over the last three years, 3) how long mycobacteriology testing was interrupted by each event, 4) revisions or additions to COOPs, especially for the mycobacteriology laboratory after these events, 5) lessons learned from these events for future preparedness and 6) communication of the importance of revising COOPs after these types of events to address the continuation of testing services specifically mycobacteriology testing.
In addition, this assessment will also increase awareness of the importance of having a COOP in place for the continuation of essential services such as mycobacteriology testing and guide the development of future Laboratory Branch resource activities dedicated to maintaining a strong infrastructure for mycobacterial testing.
Data will be kept secure throughout the analysis and reporting process.
The questionnaire should take no more than 10 minutes to complete. Please use this link to access the questionnaire: https://airc.cdc.gov/surveys/?s=8R974XWXXJ.
Note: only one questionnaire may be completed per laboratory. If you cannot participate or believe another member of your laboratory staff is better suited to complete the questionnaire, please forward this link to that individual.
Please complete the assessment by [INSERT DATE]. (2 week)
If you have any questions, you can contact Stephanie Johnston, Team Lead, Laboratory Capacity Team at [email protected] or 404-639-5019.
Thank you for your participation,
The Laboratory Capacity Team
