Dear Online ACAS Participant,
Thank
you for agreeing to participate in the ATSDR Communication Activities
Survey (ACAS) regarding (site).

The survey will give us feedback on how effective and timely ATSDR is in informing communities. It will also help public health staff improve the work they do in a way that’s best for community members by providing valuable health information so that you may protect yourself, your family, and your community from environmental health risks.

Your responses and identity will be kept private to the extent allowed by law. Digital data will be stored in secure computer files. We will not release your identity in any ATSDR reports.
