Purpose, Privacy Act Statement and Burden Information
The U.S. Department of State, Bureau of Diplomatic Security, Office of Personnel Security and Suitability routinely
conducts quality assurance inquiries of our investigators' performance to measure their level of effectiveness, efficiency,
and professionalism while conducting Department of State background investigations. Our records indicate you were
interviewed by one of our investigators, either as the subject of investigation or as a source of information about
someone else. Please help us assess our performance by responding to the following questions regarding your
interview.
This information will be used to measure investigator effectiveness, efficiency and professionalism while conducting
U.S. Department of State personnel security background investigations. The information provided is voluntary and your
failure to respond will not affect the investigation to which this inquiry pertains.
Public reporting burden for this collection of information is estimated to average three minutes per response, including
time required for searching existing data sources, gathering the necessary documentation, providing the information
and/or documents required, and reviewing the final collection. You do not have to supply this information unless this
collection displays a currently valid OMB control number. If you have comments on the accuracy of this burden estimate
