Information includes sufficient personally identifiable information and/or source documents as applicable. Failure to provide the required information may result in no action being taken on the request. Authority to collect this information is contained in 5 U.S.C. 552, 5 U.S.C. 552.

Short Statement: Only the Certification of Identity Form is being used. The system would not allow for the deletion of the other IC listed, so I removed the burdened hours associated with it instead. The Certification of Identity Form's burden was increased to reflect the uptick in FOIA requests received on an annual basis.
