The President and Chief Executive Officer of the operating cruise company, the Chief Ethics and/or
Compliance Officer of the operating cruise company and all parent companies, and the highest-ranking
Medical Officer of the operating cruise company and all parent companies must sign the following
attestation:

IN LIEU OF A SIMULATED VOYAGE

In accordance with 18 U.S.C. § 1001, I do hereby certify that the following conditions are true and
correct to the best of my knowledge and belief for _________________________________ [insert

A clear and specific vaccination plan and timeline to limit cruise ship sailings to

I understand that any false statement made in connection with the submission may subject me to
criminal penalties under 18 U.S.C. § 1001. I further understand that this attestation is being submitted
