What is this collection about? |
The information is used by the Coast Guard (CG) to ensure that personnel aboard ships are made aware of the proper usage and stowage instructions for certain hazardous materials. Provisions are made for waivers of products in special Department of Transportation (DOT) hazard classes. |

When must information be submitted to the Coast Guard? |
Information is submitted only when an owner, operator, supplier or manufacturer (applicant) seeks approval of an unusual ships’ store. |

In writing or electronically via e-mail. Information is submitted to the CG. Contact info is below. |
What happens when complete information is received? |
CG will review the information and determine if the hazardous material may be on board as ships’ stores. CG will advise the applicant in writing. |
