In an effort to place all of the requested information on page one, the changes were made since approval on August 6th:
Checkboxes added in the PDF version.
The title was indented to make room for agency seal.
Moved the footnotes to the body and removed the formatting from what had been the first footnote.
Redid the signature section to take up less space by removing the first and last words from “your printed name here” and “your signature here.”
OPM is not responsible for government wide SORN coverage for contractors and visitors – only for employees under OPM Gov’t 10 there are 2 forms – one for employees and one for contractors/employees/visitors.
