Sheet 1: Instructions

WHAT'S INCLUDED IN THIS WORKSHEET?

This excel file contains two tabs -- one to capture the FLAGSHIP prevention EFFORT on your installation in seven key attention areas (Column A - attention area); the other tab is to capture the number of key personnel that are available to contribute to prevention of self-directed harm, abusive, and harmful behaviors on your installation.

FLAGSHIP EFFORT INSTRUCTIONS:

If possible, please identify ONE flagship effort your installation is doing in each of these areas and complete columns C-K for this effort. A flagship effort is the most important, impactful, and crucial effort your installation is doing in each of these attention areas. There is also an option to include on other flagship prevention effort (marked in Column A - attention area as the Other please specify) The other tab is to capture key information about the personnel contributing to prevention at your installation.

WORKFORCE INSTRUCTIONS:

Please indicate the number of personnel authorized to and assigned to support each mission area (Column A). The worksheet also asks for the percent of these personnel that are dedicated full-time to the mission area, and for some information on training and professional development for these personnel. The personnel should all dedicated to the installation and installation efforts, and should not include any Service Branch or HQ personnel.

Sheet 2: Flagship Effort
