Abstract: As required by Executive Order 14042 USAID is implementing the federal COVID-19 vaccination mandate and evaluating compliance by Personal Services Contractors (PSCs). In order to determine compliance, USAID must collect medical information including the number of vaccination doses received, dates, and type of vaccination. Respondents will be USAID PSCs.

Emergency Justfication: USAID must evaluate PSC vaccination documentation to ensure that the information provided complies with Executive Order 14042. USAID proposes to utilize a Report Vaccination Form to collect medical information that will help determine whether PSCs are in compliance with the federal COVID-19 vaccination mandate. The template form is the same as the form used by U.S. Direct Hire (USDH) staff and ensures USAID has the most accurate and current information on contractors’ vaccination status.
