Justification for Change Request

This is a nonmaterial/non-substantive change request for OMB No. 0920-0134, expiration date 03/31/2022. All requested changes represent minor modifications to already-approved instruments including revised formatting, rewording, new answer options, and the addition/subtraction of a limited number of questions.

This non-substantive change request is to notify OMB that we will no longer be using the form “Air Travel Illness or Death Investigation Form” as part of this information collection. A modified version of this form is being used under 0920-1318 and the burden hours associated with using that form are reflected in that information collection.
