Justification for Change Request
This is a nonmaterial/non-substantive change request for OMB No. 0920-0134, expiration date 03/31/2022. All requested changes represent minor modifications to already-approved instruments including revised formatting, rewording, new answer options, and the addition/subtraction of a limited number of questions.
This non-substantive change request is to modify the form “Application for Special Exemption for a Permitted Dog Import” to include photos of the face and side of body on the permit. The form previously only included photos of the dog’s teeth (front and side).
