Forms for this collection are
completed by panel physicians for refugees and aliens seeking
immigrant visas to the U.S. The collection records medical
information necessary to determine whether refugees or immigrant
visa applicants have medical conditions affecting the public health
and requiring treatment.
Subject to certain
exemptions and waivers, under Section 212(a)(1)(A)(ii) of the
Immigration and Nationality Act, individuals are ineligible for
immigrant visas unless they have been inoculated for
vaccine-preventable diseases recommended by the Advisory Committee
for Immunization Practices. Immigrant visa applicants, as well as
refugees and some nonimmigrant visa applicants, must complete a
medical examination before a panel physician before a consular
officer may issue the visa. Panel physicians must follow Centers
for Disease Control’s Technical Instructions for Panel Physicians
and record their findings on DS-3025 (OMB Control No.
1405-0113).
