Q1.1 Flash Flood Response Survey   Please complete the following 5-10 minute survey regarding the {DATE} rain event.   Your participation in this study is voluntary and your identity will remain anonymous. You may skip any question you don’t feel comfortable answering. You may withdraw from the study at any time without penalty. All information that you enter will remain confidential, and no information that can be linked to you will be made public. Any results from the survey will talk about groups of people and none of your individual information will be shared with anyone.  Your decision to participate will have no bearing on your relationship with any of the organizations that are mentioned here. There are no risks associated with participating in this study. There are no penalties if you choose not to participate. No monetary or other such form of financial compensation will be offered to participants of this study. Results from this study may be shared with local jurisdictions, and results will be made available to the public upon request.    This survey is funded by the National Oceanic and Atmospheric Administration. We are working to understand public response to hazardous weather events in order to improve how weather information is delivered to you. This survey should take about 5-10 minutes to complete.   If you have any questions about this survey, please contact Brenda Philips at [email protected], or Cedar League at [email protected].
