Demographic Information Collection Form

For Reviewers: Interested participants will be taken to this brief form by clicking on the link in the recruitment email. This survey is used to confirm participant eligibility and to collect participation/communication preferences. The survey will be hosted by the University of Florida research team and administered via Qualtrics. Data will be stored securely and separately from all focus group/interview data.

Thank you for your interest in our research to improve Sexual Assault Leadership Training at the U.S. Air Force Academy (i.e., the SALT Study). If you are interested in volunteering to participate in an interview or focus group, please complete the form below and a member of our research team will reach out to you with more information.
