Instrument 8: Learning Session Day 1 Evaluation

The purpose of the information collection is to get feedback on improvement team members’ experiences with Day 1 of a learning session. In-person Learning sessions will take place twice over the course of the project. The Day 1 Evaluation collects information on what was most and least helpful, the questions participants have, and any other concerns that participants want to discuss. The Fathers and Continuous Learning (FCL) team will use the information to improve the second day of the learning session and to understand participants’ reactions to the process.

It will be used both to plan learning sessions and to understand the successes and challenges of implementation for pilot study.

Public reporting burden for this collection of information is estimated to average 10 minutes per response. Providing this information is voluntary, and all responses that are collected will be kept private to the extent permitted by law.

An agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number. The OMB number for this information collection is 0970-0531 and the expiration date is 07/31/2022.
