PRIVACY ACT STATEMENT:


Authority: 10 U.S.C. 5013, Secretary
of the Navy; 10 U.S.C. 8041, Headquarters, Marine Corps; 10 USC 10212, Gratuitous services of officers: authority to accept; and
DoDI 1322.22, Service Academies.



Purpose: Collect and maintain the necessary
information to assign sponsors to midshipmen and administrate the Naval Academy Sponsor Program.



Routine Uses: Used by the Sponsor Program Office to determine if candidates are a good fit for the program, to maintain a record of the names and addresses
of
families assigned as sponsors or who are interested in the Sponsor Program, and to contact sponsors either by phone or written correspondence.



Disclosure: Voluntary; however, failure to provide the required information may result in a delay or inability to process the application.
