Abstract: This form is used by national and local non-profits, small community and faith-based organizations, government agencies, and other prospective service project sponsors in the submission of proposed service projects for consideration by AmeriCorps NCCC. The information collected by the form is used by AmeriCorps NCCC to evaluate the proposed service project for approval and selection.

Short Statement: Changes were made based on feedback from stakeholders, resulting in a decrease of 3,600 annual hours from deletion of the “Needs” and “Organizational Capacity” narrative pages. The cost per response has increased because we were not required to submit this information the last time it was approved.
