Justification
RI 38-115 – Representative Payee Survey
This information collection is being submitted as a non-substantive change. RI 38-115 is used to determine whether representative payees are using the benefit to support the annuitant.
The following edits were made to this information collection request (ICR):
(1) updated the edition date
(2) updated the final 4-digit codes of full 9-digit ZIP code
(3) page 1, added bulleted paragraphs #1, 2, 3, 4 and 5
(4) page 2, revised questions #1, 3, 4, 5 and 8. Added questions #8a, 8b, 9 - 17.
(5) changed “Beneficiary” reference to “Annuitant”.
There are no changes to the burden.
