CAB ASSESSMENT SURVEY


Thank you for attending this Community Awareness Briefing (CAB). To continually improve the CAB, we need your input. Your participation in this survey is voluntary. You may choose not to participate, and you may stop participating at any time. This survey should take less than 5 minutes to complete. You will not be asked to provide any personal information. Instead, this survey asks about your general knowledge of the topic. At the end of the CAB, you’ll be asked questions about whether your general knowledge has improved and what you thought about the CAB itself.
