Eligibility Questions

Are you 18 years of age or older?	Yes No If “no”, “Thank you for your interest in this study, but eligible participants must be over the age of 18.” If “yes”, go to next question
Are you a military service member (active duty, reserve, or Veteran status) or partner of a military service member?	Yes No If “no”, “Thank you for your interest in this study, but eligible participants must be a military service member or a partner or a military service member.” If “yes”, go to next question
Are you associated with an installation?	If “no”, “Thank you for your interest in this study, but eligible participants must be associated with an installation.” If “yes”, go to next question
What installation are you associated with?
